We Provide

We can provide the following services, tailored to fit your needs or the needs of your loved one:

  • Care Coordination
  • Nursing Services including: Medication setup   
  • Personal Care: Bathing, Grooming, Skin and Nail Care, Transferring
  • Homemaking: Meal Preparation, Laundry and Linen Changes, housekeeping and shopping assistance
  • In-Home Respite Care
  • Supplemental Services to Support the individual's specific needs 
  • Counseling, Support Groups and Trainings
  • Loan Closet with a variety of medical equipment to borrow free of charge for as long as necessary

Our Home Health Program is a community based in-home service maintained through the Buffalo Senior Center. It is designed to meet the needs of senior citizens and adults with disabilities. 

 

Personal care is supervised by a Registered Nurse (RN) and provided by Certified Nursing Assistants (CNA) who are Wyoming State Board certified Home Health Aides (HHA). Background checks are performed on all staff before entering a person’s home.

 

Access Care Coordinators (ACC) are certified by the Wyoming Department of Health’s Aging Division. These coordinators determine eligibility, develop care plans, and refer people to community resources.
